What Happens Next ?
You will receive a confirmation email to let you know that we have received your intake form. Please ensure you check your spam filters. Our staff and/or midwives will review the information provided and will communicate via phone or email typically within 2 weeks. Unfortunately we are not able to accommodate everyone who applies and there is a chance you will be placed on our waitlist. It is important that you continue your routine prenatal care with your provider until you have been scheduled your first clinical appointment.
We treat the information gathered on this intake form with strict confidentiality. Please note: if we are unable to accommodate you, we will be sharing your name, date of birth and postal code with the Ministry of Health. This information is shared to demonstrate the need for more midwives in our community. Please notify us in writing if you do not want us to share this information.
